The two problems feed into and worsen each other, which is why they need to be treated at the same time and in a coordinated way.<\/p><h2>What is dual diagnosis?<\/h2><p>These are not two independent problems that happen to coincide. In dual diagnosis, the addiction and the mental health condition are intertwined: substance use changes the course of the condition, and the condition shapes the substance use. A common example: someone with anxiety starts drinking to calm down; alcohol brings relief for a few hours, but over time it increases the underlying anxiety and creates dependence, and the person drinks more to compensate.<\/p><p>It is a common situation among people who ask for help with an <a href=\"\/en\/types-of-addictions\/what-is-addiction\/\">addiction<\/a>, and also among people seen by mental health services. Detecting it early makes a big difference to the outlook, because it avoids treatments that only look at one part of the problem.<\/p><h2>Conditions most often linked to addiction<\/h2><ul><li><strong>Anxiety disorders:<\/strong> generalised anxiety, panic attacks or social phobia, and also obsessive-compulsive disorder.<\/li><li><strong>Mood disorders:<\/strong> depression and bipolar disorder.<\/li><li><strong>Psychotic disorders:<\/strong> such as schizophrenia.<\/li><li><strong>Personality disorders:<\/strong> particularly borderline and antisocial personality disorder.<\/li><li><strong>ADHD:<\/strong> attention deficit hyperactivity disorder, including in adults.<\/li><li><strong>Post-traumatic stress disorder:<\/strong> after experiences such as violence, abuse or serious accidents.<\/li><\/ul><p>Any addiction can be part of a dual diagnosis: alcohol, cocaine, cannabis, psychotropic medication, opioids or non-substance addictions.<\/p><h2>Which comes first, the addiction or the mental health condition?<\/h2><p>It can happen in either order:<\/p><ul><li><strong>The mental health condition first:<\/strong> the person starts using to relieve sadness, distress, insomnia or other symptoms. This is often described as \u2018self-medicating\u2019.<\/li><li><strong>Substance use first:<\/strong> continued use of a substance triggers or brings out a condition, as happens with some forms of anxiety, depression or psychosis associated with cannabis or stimulants.<\/li><li><strong>Shared roots:<\/strong> genetic predisposition, traumatic experiences, ongoing stress or social exclusion can contribute to both problems at once.<\/li><\/ul><p>Knowing the order helps to understand each person\u2019s story, but in every case treatment must address both conditions.<\/p><h2>Signs that may point to dual diagnosis<\/h2><ul><li>Using in order to \u2018function\u2019: to sleep, calm your nerves, lift your mood or concentrate.<\/li><li>Emotional distress does not improve, or gets worse, when trying to stop using.<\/li><li>There have been separate addiction or mental health treatments without lasting improvement.<\/li><li>There are sudden mood swings, extreme mistrust, strange perceptions or thoughts of death.<\/li><li>Relapses coincide with emotional crises.<\/li><\/ul><p>If there are thoughts of suicide or an immediate risk, call <strong>112<\/strong>, the European emergency number. You can also call 024, Spain\u2019s suicide prevention helpline.<\/p><h2>How it is diagnosed<\/h2><p>The diagnosis should be made by a team trained in both addiction and mental health. It is not always straightforward: the effects of a substance, or of withdrawal from it, can resemble those of a mental health condition, and vice versa. That is why the assessment usually includes:<\/p><ul><li>a detailed interview about substance use, symptoms and how they have developed;<\/li><li>personal, family and medical history;<\/li><li>observing how the symptoms change when substance use is reduced or stopped;<\/li><li>if needed, additional tests and information provided by the family.<\/li><\/ul><h2>Dual diagnosis: integrated treatment<\/h2><p>The recommended approach is <strong>integrated treatment<\/strong>: the same team, or very closely coordinated teams, address the addiction and the mental health condition with a shared plan. When only one part is treated, the person often ends up caught between services and relapses become more likely.<\/p><h3>What it usually includes<\/h3><ul><li><strong>Psychiatric care:<\/strong> assessment and, if indicated, medication for the mental health condition, adjusted to the person\u2019s substance use.<\/li><li><strong>Psychotherapy<\/strong>, individual and group, to understand the role substance use plays, manage emotions and prevent relapse.<\/li><li><strong>Supervised detoxification<\/strong> when necessary. Alcohol and benzodiazepines should not be stopped suddenly without medical supervision.<\/li><li><strong>Work with the family<\/strong>, who need information and support to help without wearing themselves out.<\/li><li><strong>Social support<\/strong> with housing, employment or relationships when the situation requires it.<\/li><\/ul><p>The intensity is adapted to each stage: treatment can be outpatient or, at the most difficult times, take place in a residential service.
